PROGRAM OVERVIEW

Since 2015, the Hospital Medicine Summit (HMS), a multidisciplinary educational event, has been providing an exceptional educational experience that is inclusive of clinicians who manage patients in the acute setting and those who have transitioned to the chronic-care setting. Serving hospitalists, internal medicine physicians, physician assistants, nurse practitioners, pharmacists, and nurses, HMS fulfills a critical need to address significant changes in the healthcare system and the increased demand on these clinicians so they can provide optimal care to patients both in the hospital or acute care setting and when they are discharged into the community healthcare system.

TARGET AUDIENCE

This educational activity has been designed to meet the needs of physicians, physician assistants, nurse practitioners, registered nurses, and pharmacists who practice in hospital, internal medicine, and chronic-care settings.

LEARNING OBJECTIVES

Keynote: Collaborative Care Between Hospital-based Clinicians and Community Specialists

  • Learning objectives to come

Session 1 – Taking the Kidneys to Heart: Managing CKD-Related Anemia With Emerging HIF-PHIs

  • Clarify the disease burden imposed by untreated or undertreated anemia in patients with chronic kidney disease (CKD), including in racially diverse populations
  • Correlate the pathophysiology of anemia in CKD with viable therapeutic targets
  • Identify appropriate candidates for emerging CKD-related anemia treatments among patients on or not on dialysis based on efficacy and safety data from recent clinical trials

Session 2The Role of the Hospitalist: A Focus on Oncology Treatment Adverse Events

  • Review the associated grade <3 treatment-related adverse events (trAE) with new and novel anticancer agents
  • Identify patients presenting with serious trAEs and devise an appropriate management plan

Session 3 Fireside Chat* Faculty Panel Discussion

  • Correlate the pathophysiology of anemia in CKD with viable therapeutic targets
  • Identify patients presenting with cancer trAEs and devise an appropriate management plan
  • Discuss how to implement a plan of care for patients hospitalized with hepatic encephalopathy (HE) that is consistent with updated evidence-based recommendations

Session 4 – Evidence-based Care and Therapeutic Updates to Reduce the Risk for Rapid Progression and Recurrence of Hepatic Encephalopathy

  • Discuss how to implement a plan of care for patients hospitalized with HE that is consistent with updated evidence-based recommendations
  • Describe the efficacy and safety of therapies for treatment of acute episodes of overt HE and prevention of their recurrence
  • Identify crucial components of a transitional care plan to prevent future hospitalizations among patients with HE
