Since 2015, the Hospital Medicine Summit (HMS), a multidisciplinary educational event, has been providing an exceptional educational experience that is inclusive of clinicians who manage patients in the acute setting and those who have transitioned to the chronic-care setting. Serving hospitalists, internal medicine physicians, physician assistants, nurse practitioners, pharmacists, and nurses, HMS fulfills a critical need to address significant changes in the healthcare system and the increased demand on these clinicians so they can provide optimal care to patients both in the hospital or acute care setting and when they are discharged into the community healthcare system.
This educational activity has been designed to meet the needs of physicians, physician assistants, nurse practitioners, registered nurses, and pharmacists who practice in hospital, internal medicine, and chronic-care settings.
